Where We Work
We work with those age 65+ residing at home in Palatine, Rolling Meadows, Mount Prospect, Arlington Heights, Buffalo Grove, Des Plaines, and all towns located inside Palatine and Wheeling townships. We are working to expand our areas of coverage, and the best way to do that is by spreading the word!

How We Work
The occupational therapist, nurse, and handyman come to the home to deliver services over a four month period. In most cases, the occupational therapist makes 6 visits, and the nurse makes 4 visits.
1. Initial screening and interview with North West Housing Partnership
2. 1st visit with Occupational Therapist - includes assessment and discussion of priority goals. This visit, as with all subsequent visits, takes place in the patient's own home
3. 2nd visit with Occupational Therapist - includes examination of the home and creating a work order for the Handyman
4. Handyman comes to make repairs and, if needed, modifications to the home to make it more accessible
5. 1st Nurse visit - focused on pain, strength, medication, and communication with health care providers
6. Additional Occupational Therapist visits to address priority goals
7. 2nd Nurse visit to set goals, review exercises, and consider how to improve communication with the person's primary care provider
8. Additional Nurse visits to review progress and use strategies, and complete the action plan
9. The 6th Occupational Therapist visit is a wrap-up and helping the participant generalize what they learned, so that they can address future challenges more easily. If someone picks only 2 goals, sometimes they have just 5 visits
10. Following the completion of service, there will be a survey and evaluation visit to answer questions and assess impact

Who Is EligibleAge 65+ , residing at home in Palatine, Rolling Meadows, Mount Prospect, Arlington Heights, Buffalo Grove. Be willing to answer questions before the services begin as well as after services are completed. Agree to work with an occupational therapist (OT) and nurse on agreed-upon gals. These professionals are affiliated with Attuned Care which supports senior communities in optimizing health outcomes. The handymen are affiliated with North West housing Partnership.

How much does it cost?
It's FREE! Thanks to grants from CAPABLE Program Partners, this program is free to its participants. These grants do have end dates, and how the program will be covered under Medicare is not fully determined yet. So, take advantage of the amazing program while you can and get the word out so that CAPABLE can continue to provide these free services in the Chicagoland area for years to come.
